Summary

The facility failed to maintain complete and accurate documentation for one resident. The resident had diagnoses including high blood pressure, chronic pain, and a personal history of traumatic brain injury. Facility documentation included a nursing progress note stating the resident was in the hallway when an active shooter fired a gun, that he was taken to safety, had a skin assessment, and that police and a family member were notified. A later nursing note clarified that the resident stated he heard a loud noise he believed sounded like a gunshot, got out of bed into his wheelchair to investigate, and saw an unfamiliar female near the elevator with what appeared to be a pistol at her waist. The resident’s record also contained subsequent documentation showing ongoing emotional distress related to the incident. Social Services documented that he reported flinching when approached, intrusive images of the person with a weapon, inability to sleep, anxiety, hypervigilance, and worsening symptoms despite psychiatry visits and medication adjustments. CRNP notes later documented depression, anxiety, insomnia, and ongoing fear related to loud noises and the incident, with medication changes made for those symptoms. However, the clinical record did not include additional documentation from Psychiatry or Psychology services that staff expected to be present. During interviews, staff stated they expected to see counseling notes and psychiatric discussion of the incident, but those records were not in the chart. A Social Worker and CRNP later produced paper copies of Psychology and Psychiatry notes that had not been uploaded into the resident’s clinical record. The Psychology note stated the resident reported feeling fine and declined future services, while the Psychiatry note stated he was stable and had increased sensitivity to loud noises. Staff confirmed the notes had been sent by email and were supposed to be uploaded, but were not maintained in the resident’s record.
