Infection Prevention and Control Lapses During Medication Pass

Summary

The facility failed to prevent the potential spread of infection during medication administration for two residents and failed to maintain an environment free from the potential spread of infection on the 300-unit. Review of the hand hygiene policy showed staff were to use alcohol-based hand rub or soap and water before and after direct resident contact, before preparing or handling medications, after contact with objects such as medical equipment, in the immediate vicinity of a resident, and after removing gloves. On June 9, 2026, the freezer in the 300-unit pantry refrigerator was observed storing ice cream along with two medical-type ice packs labeled with resident names. An LPN stated the ice packs had been used by residents for medical purposes such as pain relief and reduction of inflammation/swelling and were not for food service use. During observation of a medication pass on June 10, 2026, an LPN handled medications for one resident by reaching ungloved fingers into an open stock bottle to obtain aspirin and vitamin D tablets, then entered the resident’s room without performing hand hygiene. After the resident requested stool softeners, the LPN administered the initial medications and left the room. Another nurse then prepared polyethylene glycol powder and Senna S tablets and returned to administer them without performing hand hygiene. The first LPN later returned to the cart, prepared furosemide and zinc sulfate without hand hygiene, and re-entered the room to administer those medications. The same LPN then moved directly to another resident, gathered blood glucose testing supplies without hand hygiene, applied gloves, entered the room, performed a finger stick, discarded the soiled test strip wrapped in the used glove into the resident’s trash can, and exited to prepare insulin without performing hand hygiene. During interview, the LPN confirmed the observations, stated hand hygiene was not performed after the blood glucose check because the sink was in use, acknowledged sanitizer was available on the medication cart, and stated they were not aware the used glucose test strip was medical waste requiring appropriate disposal.
