Summary

The facility failed to provide nursing services consistent with professional standards of quality by not ensuring licensed nurses administered medications in accordance with physician-ordered parameters for one resident. The resident was admitted with hypotension and had a quarterly MDS showing moderate cognitive impairment with a BIMS score of 11. A physician order dated January 26, 2026 directed staff to give Midodrine HCl 5 mg orally three times daily and to hold the medication if systolic blood pressure was greater than 120 mm/Hg. Review of the May and June 2026 MARs showed nursing staff administered Midodrine HCl on 15 occasions even though documented systolic blood pressure readings exceeded the ordered hold parameter. The recorded blood pressures at the times of administration included readings such as 150/80, 134/68, 144/76, 150/82, 142/68, 158/82, 146/76, 142/90, 126/80, 130/82, 136/90, 126/68, 130/82, and 137/80 mm/Hg. The Nursing Home Administrator and DON reviewed these findings during an interview on June 15, 2026, regarding the facility's failure to ensure nursing staff administered blood pressure medication according to physician-ordered parameters.
