Unsanitary food storage, dishwashing, and ice machine conditions

Summary

The facility failed to maintain sanitary food storage practices in the food and nutrition services department. During an initial tour, cardboard food containers stored on metal shelving were observed to be wet and covered with freezer burn. A large accumulation of ice was also observed on the freezer ceiling, shelving beneath the exhaust fans, storage shelves, food containers, the floor, and the door threshold. The freezer temperature measured 9 degrees Fahrenheit, and the Certified Dietary Manager was unable to state whether the freezer had been consistently maintained at acceptable temperatures or whether the excessive ice accumulation had been ongoing. The facility also failed to maintain sanitary dishwashing and ice machine sanitation practices. On two separate observations, dietary aides loaded soiled dishes into the dishwasher while wearing gloves and then removed clean dishes from the dishwasher using the same gloves without removing them or performing hand hygiene. In addition, both ice machines observed in the dining room/activity rooms had unsanitary conditions around them, including lint on an external filter, dried liquid residue on chrome surfaces, debris and stains on the floor, and brown sticky residue, dirt, staining, and black spots on drainpipes and nearby piping. The Nursing Home Administrator confirmed the facility was expected to maintain safe food storage practices and sanitary conditions for equipment used in the preparation and service of food and ice.
