Infection Control Deficiency Related to PPE Use and Resident Care

Edenbrook Of YeadonYeadon, Pennsylvania Survey Completed on 04-23-2026

Summary

The facility did not implement an effective infection control program related to PPE use and wound/tube-feeding related care for two residents. The facility policy on Isolation Precaution required hand hygiene and gloves and gown before entering a contact precaution room, and removal of gloves with hand hygiene before leaving. Resident R2 had diagnoses including dysphagia and gastrostomy status, was receiving PEG tube feeding, and his care plan required head-of-bed elevation during and 30 minutes after tube feeding. During observation, R2 was on enhance barrier and contact precautions, was lying flat in bed while tube feeding was running, was coughing with a gurgling sound, and had a tracheostomy mask off to the side with yellowish-beige substance mixed with mucus on his chest and tracheostomy dressing. During the same observation, an employee entered R2's room without PPE, and another employee left R2's room to retrieve a pulse oximeter from the hallway medication cart without removing PPE worn inside the room. For Resident R5, the room also had enhance barrier and contact precaution signage posted and PPE supplies outside the room, but a hospice aide was observed going in and out of the room without PPE. The aide stated she was not aware she had to wear PPE and thought the contact precaution applied only to bed A. A licensed nurse confirmed that the aide was in R2's and R5's rooms without wearing PPE and that she did not know PPE was required when entering those rooms.
