Summary

The facility failed to assess, monitor, and treat an existing pressure injury for one resident, resulting in actual harm. The resident was admitted with diagnoses including sepsis and MRSA, and the admission skin evaluation documented an open area to the coccyx measuring 1 cm by 2 cm by 0.1 cm. A care plan was initiated for impaired skin integrity, and physician orders were in place for wound cleansing and application of Calmoseptine every shift and as needed, along with weekly skin assessments. However, from admission through May 12, 2026, the clinical record contained no documented wound assessments, measurements, reassessments, or weekly skin assessment findings for the coccyx wound. Although staff documented completion of the ordered wound treatment on the TAR, there were no corresponding wound assessments in the record to show whether the wound was improving or worsening. Staff also documented completion of weekly skin assessments, but no assessment findings were entered in the assessment section of the chart during the same period. In addition, staff were assigned to turn and reposition the resident every shift, but documentation was missing for 13 of 42 reviewed shifts in April and 26 of 72 reviewed shifts in May. The resident’s consent for wound care services was signed on April 21, 2026, but the resident was not evaluated by the wound care specialist until May 12, 2026. When the wound specialist evaluated the resident, the coccyx wound had progressed to an unstageable pressure injury measuring 3.5 cm by 7.5 cm with depth unable to be determined, with slough, granulation tissue, moderate bloody and purulent drainage, and malodor. A follow-up wound clinic visit documented further enlargement of the wound and sharp excisional debridement. A wound culture later identified multiple organisms including MRSA and Pseudomonas aeruginosa, and the resident was transferred to the hospital for evaluation of the infected wound and possible osteomyelitis. The resident later required hospitalization for the infected pressure injury and UTI, transfer to another facility for a higher level of care, and surgical intervention including sacral debridement and negative pressure wound therapy. The facility failed to assess and monitor the pressure injury, failed to complete ordered skin assessments, failed to consistently document pressure-relieving interventions, failed to identify and respond to wound deterioration, and failed to ensure timely wound care treatment.
