Food Storage and Hand Hygiene Deficiencies

Yorkview Nursing And RehabilitationYork, Pennsylvania Survey Completed on 06-26-2026

Summary

The facility failed to store and serve food and beverages in accordance with professional food safety standards in three of five pantry refrigerators and during the lunch meal service. In the Subacute nourishment pantry, surveyors observed a half-eaten strawberry milk shake and fruit yogurt bowl in the freezer that were not date marked or labeled with a resident identifier, and an open 32 oz container of honey thick milk in the refrigerator that was partially used and not marked with an open or use-by date. In the F-east and west nourishment pantry, two bowls of applesauce were not date marked, and the refrigerator contained a clear liquid in the lower bin and a dried tan liquid on the bottom shelf. In the [NAME] II nourishment pantry, two thawed strawberry Mighty Shakes were not marked with a pull or use-by date, and an open 32 oz carton of Med Pass 2 was partially used and not marked with an open or use-by date. The Food Service Director confirmed that resident items should be marked with a resident identifier and date, open items should be dated, and refrigerators should be clean. During the lunch meal, a Dietary Aide with a gloved hand opened the proofing box, retrieved a hamburger roll, touched the roll to her bare arm, then used the same glove to reach into the pan of hamburgers, place the hamburger in the bun, and serve the sandwich without completing hand hygiene. Another Dietary Aide was observed serving soup and puree items with gloved hands, then picking her teeth, touching inside her mouth, and rubbing her nose with the same glove before returning to service without changing gloves or completing hand hygiene. The Food Service Director and the NHA both confirmed that the staff should have changed gloves and completed hand hygiene.
