Summary

The facility failed to convey funds credited to a deceased resident’s family after the resident died. Review of the resident trust fund policy dated 1/16/26 indicated the facility was to maintain a system ensuring full, complete, and separate accounting of each resident’s account funds. Closed Resident Record CR3 showed the resident was admitted to the facility and had diagnoses on the MDS assessment dated 9/21/25 that included lung cancer, diabetes, and hyperlipidemia. A clinical nurse note dated 10/30/25 documented that staff were called to the resident’s room by family, assessed her, and found she had no apical pulse, no respirations, and fixed, dilated pupils; she was pronounced dead at 9:28 a.m., with hospice and the DON notified and her daughter and son-in-law present. Review of business office documents dated 5/28/26 showed Closed Resident Record CR3 had a credit of $650 owed to her family. However, review of the resident’s clinical records and business office records did not show that the credit was issued to the family. During an interview on 5/28/26 at 11:56 a.m., the BOM confirmed that the facility failed to convey the funds credited to Closed Resident Record CR3’s family as required.
