Failure to Use PPE for Resident on Contact Precautions

Summary

The facility failed to ensure staff wore PPE for a resident on contact precautions. Resident 2 was admitted in 12/2024 with a diagnosis of parkinsonism. On 6/1/26, Resident 2's door had a contact precaution sign and a PPE storage container outside the room, but the signage did not include instructions on what to put on before entering the room or where to doff PPE before exiting. The instructions for donning and doffing PPE were inside the drawer of the PPE bin. On 6/1/26 at 10:46 AM, Staff 10, an LPN, stated Resident 2 was on contact precautions due to vomiting, loose stools, and stomach cramps, and that everyone was required to don PPE before entering the room. Later that day, Staff 10, a housekeeper, was observed inside the resident's room about two feet from the resident without a gown or mask, then exiting and re-entering the room to remove trash and place a liner in the trash can. Staff 10 later stated she had just cleaned the resident's toilet and bathroom, did not know what the contact precautions sign meant, was unsure who to ask, and was unaware of what to do when the sign was on the door. The Environmental Services Manager stated housekeeping staff were expected to refer to and follow the signage for PPE, and the Administrator and DNS acknowledged Staff 10 did not don PPE before entering the room.
