Summary

The facility failed to ensure residents had access to the grievance procedure and the ability to file an anonymous grievance. During a tour of the facility, there was no information posted regarding the grievance procedure or the location of grievance forms. Grievance forms were later found in the copier room in an open cabinet on the wall by the DON's office and in a file cabinet at the nurse's station with other facility forms, with no information indicating the forms were located there. The DON stated the copier room was not locked and residents had access to the grievance forms, but also stated a resident in a wheelchair would not have access to the forms. The facility's Grievance Policy, dated 01/2024, stated residents would be informed orally and in writing of their right to make complaints and grievances and the process to do so during admission, readmission, and the care planning process. However, CNA #1 stated they had not seen any posted grievance process in the facility. The social services director, who stated they were responsible for grievances, could not locate the facility's grievance process or identify who the grievance official was in the admission agreement, and was not sure if a resident could file an anonymous grievance. The social services director also stated the grievance procedure was not posted, and the administrator was unable to locate a posted facility grievance procedure.
