Summary

The facility failed to ensure medications were administered as ordered for three residents reviewed for medication administration. For one resident with diagnoses including cerebral infarction, chronic respiratory failure, COPD, CHF, anxiety, atrial fibrillation, and CKD, the record showed ordered evening medications of atorvastatin, famotidine, fluoxetine, and topiramate, along with morning metoprolol and omeprazole, were not administered on the admission date even though the medications were stocked in the automatic medication dispensing system. The DON verified the nurse did not administer the medications and stated the medications were available in the dispensing system and should have been pulled from there. For another resident with quadriplegia, autonomic dysreflexia, neurogenic bladder and bowel, anxiety, and contractures, testosterone 200 mg/mL was ordered weekly, but the MAR showed it was administered on multiple dates, including one administration four days after the prior dose. The DON verified the medication was not administered as ordered because it was backordered on one scheduled date and then later given again by mistake. For a third resident with Parkinson's disease, bipolar disorder, insomnia, anxiety, depression, hallucinations, tremors, and muscle weakness, carbidopa-levodopa was ordered multiple times daily, but medication audits showed several doses were given late, including doses administered hours after the scheduled times and one dose given again only 1 hour and 10 minutes after the prior dose. The resident's family member reported the medication was not always given timely and that the resident was visibly slowed when doses were delayed; the DON verified the findings.
