Summary

The facility failed to ensure resident family members and other individuals were able to contact facility staff members by telephone. Resident #93, who was cognitively intact and dependent on staff for activities of daily living, had diagnoses including Parkinson's disease, bipolar disorder, insomnia, anxiety, depression, other hallucinations, lack of coordination, panic disorder, tremors, and muscle weakness. The concern involved the resident's family member, who reported repeated difficulty reaching the facility by phone during the evening and night because the phone would ring without being answered. The family member stated the issue had been brought to the facility's attention multiple times and reported trying to call about a time-sensitive medication that the resident had not received on time on multiple occasions. Staff interviews confirmed that on the night shift, incoming calls were routed to telephones at the nursing stations, where nurses and aides were responsible for answering them. Staff reported that when nurses were administering medications or away from the nursing station, calls were often not answered, and several staff stated there were no cordless phones available and that the phones could be difficult to hear from the nursing station. The employee handbook stated staff were to answer the facility's phone promptly.
