Inadequate staffing and delayed call light response
Summary
The facility failed to provide adequate nursing staff every day to meet resident needs and failed to have a licensed nurse in charge on each shift, as shown by multiple resident interviews, staff interviews, call light tracking records, complaint intakes, and Resident Council minutes. The deficiency affected seven residents reviewed for staffing concerns and had the potential to affect all residents in the facility, which had a census of 89. Residents and staff repeatedly reported prolonged delays in response to call lights and assistance with ADLs, and the facility’s own expectation for call light response was 15 to 20 minutes or less. Resident #41 had diagnoses including acute respiratory failure with hypoxia, muscle weakness, and unsteadiness on feet, and was assessed as frequently incontinent of bladder and always incontinent of bowel. The resident’s care plan included assistance with toileting and incontinence care, yet the resident stated she could not remember the last time she received incontinence care. During observation, the resident’s brief was saturated, and a CNA confirmed this was the first time she had been in the room since starting the day shift and that the last incontinence care had been on night shift. The facility policy stated incontinent residents should receive appropriate treatment and services to prevent urinary tract infection and restore continence to the extent possible. Resident #14, Resident #51, and Resident #93 each reported delayed responses to call lights, and the call light logs documented repeated response times far beyond the facility’s expected timeframe. Resident #14 had 86 call light occurrences over 20 minutes, including 22 over 45 minutes and up to 2 hours and 29 minutes. Resident #51 had 24 occurrences over 20 minutes, including eight over 45 minutes and up to 1 hour and 13 minutes. Resident #93 had numerous delayed responses, including times ranging from 39 minutes to 2 hours and 49 minutes on multiple dates. Other residents also reported long waits for assistance, and staff described shortages on both day and night shifts, including assignments that required aides to cover both the secured memory care unit and another hallway, making it difficult to answer call lights, complete checks, and provide incontinence care in a timely manner. Complaint intakes and Resident Council minutes also reflected concerns that staffing was not sufficient and that call light response was not as prompt as usual.
