Summary

The facility failed to ensure quarterly care conferences were offered and completed for Resident #34. Resident #34 was admitted on 05/08/23 with diagnoses including anxiety, insomnia, diabetes mellitus, chronic pulmonary disease, and heart failure. The care conference documentation showed conferences on 06/30/25, 11/30/25, 01/22/26, and 04/22/26, but the record contained no evidence of a care conference being held for five months until 11/30/25. Resident #34 stated during interview that the facility had not held care conferences on a quarterly basis, and the Social Service Director and Regional Director of Clinical Services confirmed the missing care conference from 07/2025 to 10/2025. The facility policy stated the IDT team shall ensure each resident has consistent and routine care conferences. The facility also failed to timely update Resident #12's care plan. The record showed a psychiatric referral on 05/08/26 and first psychiatric services visit on 05/19/26, but the care plan contained no goals or interventions for symptom monitoring or management related to anxiety, depression, or schizophrenia, and no interviews for side effect monitoring for antianxiety, antidepressant, or antipsychotic medications. On 06/11/26, poison control was called after Resident #12 ingested a foreign material, and later that day the resident stated she wanted to die, planned to drink chemicals to make herself die, and had ingested body wash. The physician and psychiatric nurse practitioner were notified, and the resident was pink-slipped and sent to the emergency department for further evaluation and treatment. The care plan had last been revised on 04/17/26 and did not include the suicidal ideation or involuntary psychiatric hold.
