Failure to Follow Enhanced Barrier Precautions During Resident Care

Summary

The facility failed to ensure staff maintained proper infection control practice when providing care for residents on enhanced barrier precautions (EBP). This deficiency involved two residents, one with a suprapubic catheter and one with a history of multidrug-resistant organisms (MDRO), out of eight residents observed for EBP in a census of 91. Resident #5 was admitted with diagnoses including obstructive and reflux uropathy, retention of urine, and chronic viral hepatitis C. The resident was cognitively intact, had an indwelling catheter, and was dependent for personal hygiene. The care plan required EBP for the suprapubic catheter, and physician orders directed staff to cleanse the suprapubic catheter site with normal saline and cover it with split gauze daily and as needed, with EBP every shift. During observation, a CNA emptied the urine drainage bag while the resident was in bed but did not don an isolation gown, despite an EBP sign above the bed stating that gloves and a gown were required for care areas involving device use. The CNA emptied the bag into a urinal and then poured the urine into the toilet. Later, an LPN provided suprapubic catheter site care without wearing an isolation gown. Resident #33 was admitted with diagnoses including hypertensive heart disease with heart failure, systemic lupus erythematosus, and neuroleptic induced parkinsonism. The resident was mildly cognitively impaired, frequently incontinent of bowel and bladder, dependent for personal hygiene, and had medically complex conditions. The care plan and physician orders required EBP related to a history of MDRO. During observation, a CNA provided incontinence care while the resident was in bed but did not wash hands before entering the room and did not don an isolation gown, even though the EBP sign directed staff to clean hands before entering and leaving the room and wear gloves and gown for high-contact care activities. The CNA stated he did not wear a gown because the resident did not have a wound. The DON confirmed the resident was on EBP related to a history of MDRO, and an LPN initially stated the resident was not on EBP before reviewing the orders and confirming the EBP status.
