Summary

The facility failed to ensure a resident’s choice for bathing method and bathing frequency was honored. Resident #89 was admitted with diagnoses including spastic diplegic cerebral palsy, polyneuropathy, spina bifida, and muscle weakness, and was cognitively intact, wheelchair dependent, and dependent for showers/bathing, personal hygiene, and chair/bed transfers. The resident’s care plan and shower schedule indicated a preference for two showers per week on Mondays and Thursdays, with staff to assist with bathing, nail care, and reporting changes to the nurse. The resident also stated he wanted showers rather than bed baths and said he wanted his fingernails cleaned and trimmed. During observation and interviews, the resident’s fingernails were noted to be long, uneven, and embedded with a dark substance. The resident stated he had not been showered in two weeks and before that had gone two months without a shower, and that the facility gave him bed baths instead of showers. A CNA confirmed the resident received both showers and bed baths, but said bed baths were sometimes given when Hoyer lift pads were unavailable or because the transfer process was difficult and time-consuming. Review of the March bath/shower documentation showed scheduled bath/shower entries were not completed as documented, and the electronic record showed four bed baths with no showers documented for the month. The DON confirmed residents should receive two showers or baths per week, and the facility policy stated bathing frequency was to be based on resident choice.
