Failure to Update Fall Prevention Plan After Resident Fall

Summary

The facility failed to implement new safety interventions after Resident #42 fell, despite the resident being known to need assistance with transfers and being documented as noncompliant with asking for help. Resident #42 was admitted with hemiplegia and hemiparesis following cerebral infarction, chronic CHF, urinary retention, need for assistance with personal care, and unsteadiness on feet. The most recent quarterly MDS showed the resident was cognitively intact with a BIMS score of 15 and required at least partial assistance with transferring on and off the commode. After the fall on 06/14/26, the resident was found on the bathroom floor with bleeding and swelling under the left eye and was sent to the hospital for further evaluation. The incident report stated the resident’s call light was on at the time of the fall, but the resident was using the bathroom unassisted when they slipped and fell, striking their head. The resident later reported that they had used the call light and waited for staff, then tried to transfer from the toilet to the wheelchair alone when no staff responded, and that they slipped on liquid on the bathroom floor. Review of the care plan showed it was not updated after the fall with new interventions to prevent additional falls, and the incident form documented that care plan update was not applicable. The existing interventions were general fall precautions such as education, hazard reduction, fall risk assessments, therapy evaluation, and bed locks, but there were no interventions specifically addressing the resident’s known noncompliance with using the call light or the need for toileting supervision. Staff interviews confirmed the resident often self-propelled to the bathroom and did not use the call light until already on the commode, and the regional RN verified that no new fall interventions were added after the fall other than continued re-education.
