Summary

The facility failed to provide appropriate pressure ulcer care and prevent worsening of a wound for a resident who was admitted with multiple chronic conditions, including dementia, ischemic cardiomyopathy, hypertension, edema, urinary incontinence, and dependence on staff for all activities of daily living. The resident was non-ambulatory, had moderately impaired cognition, and was dependent on staff for turning and repositioning. A new blister-like area was first identified on the left buttock, but the initial nursing note documented only cleansing with normal saline and application of zinc, with notification of the POA and physician, and no further assessment or measurement of the wound was documented. The record also showed no treatment order entered for the new wound at that time. The wound was later documented as moisture associated skin damage to the coccyx, then progressed to an unstageable pressure ulcer and later a Stage III pressure ulcer with increasing size, slough, drainage, and tunneling. The record showed delays and gaps in wound-related interventions, including a period when the resident did not have a treatment ordered or administered for the left buttock wound, and later discrepancies in treatment administration records for ordered wound care. The facility also did not update the wound staging to reflect the ambulatory wound clinic’s assessment that the wound was a Stage IV sacral pressure ulcer, and there was no new order entered for aggressive offloading every two hours or limiting sitting to no more than two hours at a time. The report also documented failures related to ordered support surfaces and wound care supplies. A low air loss mattress was ordered but then held for safety reasons, and the Roho cushion ordered for wheelchair use was not obtained in a timely manner, while staff charted it as being in place before it was actually received. The record further showed that imaging ordered to rule out osteomyelitis was not documented as completed. Later, when wound dressings were unavailable, staff used a wet-to-dry dressing without documented physician notification, and on a subsequent observation the wound care process was contaminated when clean supplies were placed on an unclean surface and a gloved hand touched a marker before handling the dressing. The ambulatory wound clinic notes also stated the family reported the resident had not been turned aggressively on a regular basis, and the wound clinic documented a Stage IV sacral pressure ulcer with ongoing drainage and tunneling.
