Unsafe Food Storage and Unsanitary Kitchen Conditions

Summary

Food was not stored, prepared, and served in a safe and sanitary manner in the kitchen and food storage areas. During observation, surveyors found a freezer next to the grill line containing multiple items with ice crystals, including chopped carrots, fish, raw chicken, raw pork, hot dogs, sausages, and beef. The Dietary Manager verified the items had ice crystals and appeared to have freezer burn, and stated food with ice crystals should be discarded. Surveyors also observed a can opener blade with food debris on it, and the Dietary Manager confirmed it was dirty and should have been cleaned. Surveyors further observed a freezer containing nutritional supplements that were not properly frozen after the breaker had stopped working the prior night. The Dietary Manager confirmed staff had reported the freezer problem and that the supplements had melted, and she verified they should have been discarded. In the kitchen, the floors were observed to be dirty with debris and grime on the grout and floor tiles, with missing grout in the dish machine area. Air vents by the dish washing and food prep areas were also dirty with dust buildup, and an air conditioning unit above the preparation sink was leaking water into a bucket stored on the sink, with water splashing into the sink and onto clean utensils. Additional observations showed food contact and storage issues throughout the kitchen. Four knives with missing chips on the blades were stored in the clean knife rack, and two rubber spatulas had chunks missing. In the coolers, surveyors found opened and undated bologna, diced ham dated 05/01/26, open and undated shredded lettuce, an unopened bag of shredded lettuce that looked mushy and translucent, and undated sliced bologna along with sliced ham and turkey dated 5/22/26. The Dietary Manager confirmed several of these items should have been dated or discarded. Surveyors also observed a metal rack behind the steam table that was dirty, rusted, and had chipping paint. The Dietitian stated her April 2026 audit had identified concerns about the flooring and the freezer door not sealing properly or staying shut.
