Summary

The facility failed to ensure residents had a functioning call system on the 400 and 500 hallways, affecting 18 of 18 residents observed on those hallways. During observation, no visible call lights were present outside the resident bedrooms, and multiple residents were found without a functional call system. CNA #100 stated the residents used call buttons worn around their necks, but the call system was not working, there was no alert coming through staff phones, and staff were told by the DON to check residents more frequently. The DON confirmed the call system had been working on and off intermittently and verified that residents did not have an alternative way to call for help. The facility also failed to provide timely care to Resident #33, who had an admission diagnosis including cerebral infarction, muscle spasms of the back, and muscle weakness. The quarterly MDS showed impaired cognition with a BIMS score of 05, one-sided impairment, wheelchair use, and substantial assistance needed for toileting. The care plan identified the resident as needing one assist for toileting and transfers, with call light within reach and prompt response to requests for assistance, and the fall scale showed high fall risk. During observation, Resident #33 was yelling for help from the bathroom and did not have a functional call system there. CNA #100 stated she was unsure how long the resident had been on the toilet, believed the resident most likely transferred himself, and verified the call light was not functioning properly.
