Summary

The facility failed to ensure fall prevention interventions were implemented and maintained for two residents after falls. One resident had severe cognitive impairment, required extensive assistance with most ADLs, used a wheelchair, and had a history of falls including a prior fall with major injury that resulted in a left hip fracture requiring surgical repair. After a fall from attempting to self-transfer from bed to wheelchair and another fall from the wheelchair in the hallway, interdisciplinary documentation identified that wheelchair placement at bedside and grippy strips were to be added, and the care plan included bed in low position, call light within reach, nonskid footwear, assist with transfers, and grippy strips at bedside. On observation, grippy strips were not in place at the bedside as an implemented fall prevention intervention. A LPN confirmed the grippy strips were not implemented. The record also showed that after the resident’s major injury and return to the facility, the care plan continued to list the bedside grippy strips intervention, but it was not observed in place during the survey. The second resident had moderate cognitive impairment, required supervision to moderate assistance with transfers and ambulation, and was identified as a fall risk with at least one fall. After a fall when the resident attempted to retrieve glasses from the floor and slid from the bed, interdisciplinary documentation updated the care plan to include grippy strips at bedside and a reacher to assist with retrieval of items. During observation and interview, grippy strips were not in place and the reacher was not present in the room; the resident stated he did not receive a reacher following the prior fall, and the LPN confirmed the reacher should have been provided per the care plan.
