Unplugged Low Air-Loss Mattress for Resident With Multiple Pressure Ulcers

Summary

The facility failed to ensure a physician-ordered low air-loss mattress was functioning as intended for a resident with multiple pressure ulcers. The resident was admitted with a Stage III pressure ulcer to the left gluteus, a Stage III pressure ulcer to the right gluteus, and an unstageable pressure ulcer to the left heel, and also had a left elbow wound that later resolved. Admission assessments showed the resident was at risk for pressure ulcers, with a Braden Scale score of 16, and the 5-Day MDS documented two unhealed Stage III pressure ulcers, use of a pressure reducing device for the bed, and pressure ulcer dressings. The care plan included use of an air mattress and a low air-loss mattress, along with turning and repositioning, heel elevation, and skin monitoring. A physician order dated 06/01/26 directed staff to monitor the low air-loss mattress every shift and replace it if malfunctioning. During observation, the mattress pump was not operating and the electrical cord was unplugged from the wall outlet. The resident stated the mattress was unplugged and she did not know what the pump was used for, and RN #207 confirmed the mattress was unplugged and not functioning, was used for the resident's buttock pressure ulcers, and had no history of the resident unplugging it. The resident later stated she never unplugged or tampered with the mattress.
