Summary

The facility failed to ensure medications were administered as ordered for Resident #195. The resident had diagnoses including atrial fibrillation, coronary artery disease, type II diabetes mellitus, urinary retention with an indwelling urinary catheter, and protein-calorie malnutrition. After a hospitalization for sepsis, pneumonia, urinary tract infection, and abnormal laboratory findings, the resident returned to the facility with a significant decline in condition, requiring a mechanical lift for transfers, increased assistance with feeding, decreased trunk control, and therapy services before hospice was elected due to continued decline. Physician orders were entered for Lorazepam (Ativan) 0.5 mg by mouth every four hours as needed on 12/17/25 and then changed on 12/18/25 to Ativan 0.5 mg by mouth every two hours. Nursing documentation showed the resident was anxious, pulling at clothing, and reaching outward, and later had respirations of 53 and then 56 breaths per minute with ongoing end-of-life symptom management needs. The MAR showed Ativan was documented as given at 6:00 A.M. on 12/18/25, but the next documented dose was not until 4:00 A.M. on 12/19/25, despite the order for administration every two hours. During interviews, the DON stated the facility exhausted its emergency starter supply of Ativan and the pharmacy said the medication had already been packaged for routine delivery. Hospice and pharmacy staff confirmed the medication had been ordered for symptom management and that the medication remained available within the medication storage unit, but the agency nurse reported not having access to the storage unit. The facility’s medication administration policy required medications to be administered as ordered, and the medication storage access policy allowed temporary access for agency nurses under certain circumstances.
