Failure to Perform Hand Hygiene During Linen Handling and Water Service

Summary

The facility failed to implement its infection prevention and control policies for hand hygiene. The policy titled Hand Washing Requirements, effective 2/6/20, stated that hand hygiene should be performed after handling soiled equipment or utensils, after removing gloves or aprons, and after any contact with potentially contaminated objects. The policy did not mention how to handle soiled linens. During a continuous observation on 6/21/26 from 11:33 AM to 11:40 AM, NA #1 removed gloves after leaving a resident's room and discarded them in a hallway trash bin without performing hand hygiene, despite a wall-mounted hand sanitizer dispenser being nearby. NA #1 later wore gloves while gathering dirty linens from a resident's room, carried the linens into the hallway, placed them in a dirty linen bin, removed the gloves, and again did not perform hand hygiene before handling a clean washcloth from the linen cart. When asked, NA #1 stated she had not completed hand hygiene and said she was nervous and should have done so. During another continuous observation on 6/21/26 from 11:45 AM to 11:48 AM, NA #2 entered a resident's room, removed the lid from the resident's water cup with bare hands, carried the cup into the hallway to fill it with ice from an ice cooler, then returned it to the room and filled it with water from the bathroom sink before placing it on the bedside table. NA #2 repeated the same process for a second resident's water cup and did not perform hand hygiene between handling the two residents' cups or before entering the next resident's room. When questioned by the surveyor, NA #2 stated she did not complete hand hygiene but should have because germs could have been brought from one resident to the next by handling their dirty cups. The DON stated all staff were trained on hand hygiene during orientation, annually, and as needed, and the Administrator stated he expected staff to follow proper hand hygiene.
