Failure to Use PPE During Tube Medication Administration and Fingerstick Glucose Care

Summary

The facility failed to implement its infection control policies and procedures when Nurse #14 administered crushed medication through Resident #112’s gastrostomy tube without wearing a gown and gloves. The resident’s door had signage for enhanced barrier precautions directing staff to wear a gown and gloves when accessing the gastrostomy tube and suprapubic catheter, and PPE was available just outside the room. The facility’s Enhanced Barrier Precautions policy identified device care or use, including feeding tubes, as a high-contact resident care activity requiring gown and gloves. During an observation, Nurse #14 checked tube placement and administered the medication without PPE. When interviewed, the nurse stated she was new to LTC and the facility and did not think it was necessary to have so many residents with enhanced barrier precautions. The DON later stated the nurse had discussed the need for enhanced barrier precautions and PPE for gastrostomy tube access and medication administration for this resident. The Medical Director stated gloves and enhanced barrier precautions were very important to prevent infection and were required. The facility also failed to follow its fingerstick glucose policy when Nurse #14 performed care for Resident #75. After a finger stick, she exited the room holding the resident’s glucometer with a test strip containing a blood sample, without wearing gloves. She removed the strip using an ungloved hand and tissue, discarded it, and carried the glucometer back to the medication cart. The facility’s policy required clean gloves for the procedure and removal of gloves after completion. Nurse #14 stated she was new to LTC, had been there about 8 days, and thought using a tissue was an appropriate barrier. The DON and Infection Preventionist stated the nurse had received infection control education upon hire, and the Medical Director stated gloves were required.
