Summary

The facility failed to accommodate a resident’s food preferences by serving items that matched documented dislikes. Resident #109 was admitted with hypertension, had a regular diet with thin liquids ordered, and was cognitively intact on the admission MDS. The resident’s dietary tray ticket listed dislikes of pork, peppers, cheese, salt, and gravy, yet an observed breakfast tray contained bacon and an observed lunch tray contained rice with gravy. During interview, Resident #109 stated she had received pork at meals several times in the past despite her tray ticket clearly identifying her dislikes, and she said she would eat what she could and leave the disliked items on the tray. She stated nurse aides sometimes offered an alternate item when they picked up her tray, but not always, and that she had told several nurses and nurse aides she does not eat pork or greasy food. During observation of the lunch tray, the Dining Room Supervisor and Dining Room Server confirmed the dislikes were listed on the tray ticket and that the tray contained rice with gravy. The Dining Room Assistant stated staff were supposed to read residents’ dislikes aloud before plating trays and verified this was not done for the resident’s tray card. The DON stated dietary staff should verify meal trays do not include a resident’s dislike, and nurse aides delivering trays should verify tray cards and correct trays if dislikes were observed before delivery.
