Dirty Resident Rooms and Equipment
Summary
The facility failed to maintain a clean and homelike environment in 5 of 32 resident rooms on 2 of 3 halls, affecting nine residents reviewed for a safe, comfortable, and homelike environment. Observations showed heavy buildup of dirt, hair, dust, and food particles in multiple rooms, including bathrooms with dirt and hair in corners and along baseboards, rooms with debris behind beds, and surfaces with residue and grime. In one room, dried feeding formula was observed on the tube feeding pole, wall, floor, telephone, power strip, feeding pump, oxygen concentrator, and electrical cord. In another room, fall mats were sticky and covered with food particles and white granules, and the floor behind the headboards contained wrappers, packets, and dust/hair buildup. A portable fan in one resident room was observed with thick gray material covering the grills and blades, and the maintenance director stated the fan was the facility’s responsibility and acknowledged that dirty equipment was unacceptable. In another room, the metal base of an overbed table was covered with rust and sticky white residue. The observations were repeated on a second day and the same conditions remained in the affected rooms, including the dirty bathrooms, dusty baseboards, debris on floors, and residue on equipment and furnishings. Housekeeping staff stated that daily room cleaning included trash removal, floor cleaning, toilet scrubbing, and wiping surfaces, and that rooms also received monthly deep cleaning. The housekeeping director stated each housekeeper was assigned approximately 30 rooms daily and that she audited rooms weekly, but during the observations she confirmed the assigned housekeeper had completed the daily cleaning for all five rooms and stated she did not know why the rooms had not been properly cleaned. The administrator stated she was not aware of the cleaning issues, did not conduct monthly room inspections, relied on reports from the housekeeping director, and agreed the room conditions did not meet facility standards.
