Summary

The facility failed to ensure that an injury of unknown origin was thoroughly investigated to rule out abuse, neglect, and mistreatment for a resident who was found on the floor after an unwitnessed fall. The resident had severe dementia, anxiety, and a history of cerebrovascular accident, with severely impaired cognition on admission assessment. The resident was also identified as needing supervision or touch assistance for transfers and was documented as being at high risk for falls due to confusion and generalized weakness. During interviews, the CNA assigned to the resident stated they had checked the resident shortly before the fall and then went on break, and the LPN on the unit stated the assigned CNA and LPN were on break when the fall occurred. Another LPN stated they heard someone calling for help and found the resident on the floor. The RN who initiated the investigation stated they obtained statements from the assigned CNA and one LPN but did not think statements were needed from other staff on the unit at the time. The ADON/Risk Manager and the DON stated statements should have been obtained from all staff working on the unit to rule out abuse, neglect, and mistreatment.
