PASRR screening and Level II assessment not completed timely for resident with developmental disability
Summary
The facility failed to coordinate assessments with the Preadmission Screening and Resident Review (PASRR) process and to refer a resident for a Level II evaluation when the resident had a known developmental disability and the discharge plan changed from short-term rehabilitation to long-term care. Resident #140 had diagnoses including Down Syndrome and hydrocephalus, and the 11/24/2025 MDS documented intellectual disability, severely impaired cognition, and dependence in activities of daily living. The 11/18/2025 comprehensive care plan stated the resident had been living at a sibling’s home before hospitalization and was in the facility for long term care because the sibling could no longer provide care and no group home was available. The initial SCREEN Form completed on 11/18/2025 documented that the resident did not have a developmental disability and met the categorical determination of convalescent care. There was no new SCREEN Form completed to reflect the resident’s developmental disability or when the resident no longer met the categorical determination time frame. Social work notes documented discharge planning meetings with the sibling and interdisciplinary team, with the discharge destination changing to long-term care because the sibling could no longer care for the resident and OPWDD reported a group home wait list of over two years. The Level II PASRR assessment was not completed until 02/19/2026, and its purpose was documented as current nursing home placement because the resident exceeded the categorical determination time frame. After the Level II assessment was completed, there was no documented care plan reflecting the Level II PASRR determination. Social work documentation also noted that the resident had been admitted for short-term rehabilitation but transitioned to long-term care. During interviews, the Social Worker stated SCREEN forms and Level II assessments had to be completed prior to admission, and the Director of Social Work stated that if a resident was admitted with a developmental disability not reflected on the SCREEN, the facility should complete a new screen and contact the Level II coordinator; the Director also stated the Level II determination should have been reflected on the care plan.
