Summary

The facility failed to ensure drugs and biologicals were labeled in accordance with accepted professional standards for one of six medication carts reviewed, specifically the Unit 5 B Medication Cart. During an observation on 05/12/2026 at 1:37 PM with an LPN, the top drawer of the cart contained one undated vial of Humalog insulin and one expired Trelogy inhaler cartridge dated 03/25/2026. The facility policy on Storage of Medications, last reviewed 01/2026, stated that drug containers with missing, incomplete, improper, or incorrect labels were to be returned to the pharmacy for proper labeling before storing and that discontinued, outdated, or deteriorated drugs or biologicals were not to be used. During interviews, the RN Unit Manager stated that if an opened vial of medication was not dated, there was no way of knowing when it expired and it should be discarded, and that there should be no expired medications in the cart. The LPN assigned to the Unit 5 B Medication Cart stated that open vials of insulin should have the date opened and the nurse's initials, that expiration dates should be checked before administration, and that any medication without a label was discarded. The LPN also stated that the nurse who opened the medication should have written the date it was opened.
