Provide safe and appropriate respiratory care for a resident when needed was not ensured for a resident who used BiPAP therapy. Resident #3 had diagnoses including stroke with paraplegia and muscle weakness, and a hospital discharge summary documented acute respiratory failure and aspiration pneumonia. The 03/18/2026 MDS documented intact cognition, no oxygen therapy, and no BiPAP use, yet a 04/10/2026 respiratory therapy note stated the resident was ordered BiPAP at night and reported wearing it most nights, with clear bronchial breath sounds and no respiratory distress observed. There was no documented physician order or comprehensive care plan addressing BiPAP use, and the facility policy described required orders, settings, documentation, and routine cleaning of the device components. During multiple observations, the resident's BiPAP mask was seen with a significant amount of white debris inside the mask.
