Summary

The facility failed to ensure treatment and care were provided according to orders for a resident with cellulitis of both lower legs, chronic lymphedema, vascular disease, and significant edema. The resident’s care plan documented cellulitis of both legs, risk for skin breakdown related to impaired mobility, weakness, and lymphedema, with interventions including local treatments as ordered, off-loading extremities, and elevating both legs. A physician order dated 05/08/2026 directed cleansing the left and right lower leg wounds with Dakin's solution, applying methylene blue to the wound bed, covering with abdominal pads, and wrapping both extremities with Coban from toes to knees three times weekly and as needed. Observations showed the ordered dressings and compression wraps were not being provided as directed. On 05/11/2026, the resident had kerlix wrapped around both lower legs just above the feet, was barefoot, had no Coban wraps in place, and both feet and lower legs were very swollen. On 05/12/2026, the resident again had kerlix dressings on both lower legs, the right dressing was dated 05/11/2026, and there was still no Coban on either leg despite swelling. On 05/14/2026, the resident remained barefoot with kerlix dressings, the right dressing still bore the 05/11/2026 date, and the left dressing had a large amount of yellow drainage covering about 30% of the dressing. Record review and staff interviews confirmed the treatments were not completed as ordered or as scheduled. The May 2026 Treatment Administration Record showed the leg dressings were not changed on 05/13/2026. The RN Unit Manager stated nurses were responsible for completing treatments, checking dressing integrity, documenting when treatments were not done, and notifying the provider if needed. The RN who cared for the resident stated Coban was not available on the unit and that the 05/13/2026 dressing was not done because the shift ended early. The wound care RN stated the dressing dated 05/11/2026 should not still have been in place and the amount of drainage on the left dressing should have prompted a change. The NP stated treatments were expected to be completed as ordered and that they should be notified if they were not.
