The facility failed to ensure that two residents maintained acceptable nutritional status because ordered meal assistance was not consistently provided and significant weight loss was not adequately addressed. Surveyors observed that both residents were left to eat without the assistance documented in their care plans and physician orders, despite needing substantial to maximal or extensive help with meals. One resident had diagnoses including stroke with left-sided paralysis and pressure ulcers, was cognitively impaired, required substantial/maximal assistance with eating, and was on a mechanically altered diet. The resident’s care plan called for 1:1 assistance with eating and separate bowls during meals. The resident experienced repeated significant weight loss over several months, including losses of 15 pounds, 21.2 pounds, 23.6 pounds, 32.5 pounds, and other fluctuations, with several weights lacking documented reweights. The record also showed no documented evidence that the resident’s significant weight loss was addressed, and survey observations on multiple days showed the resident eating unassisted even though the tray tickets documented feeding assistance or total feed. The other resident had diagnoses including dysphagia, cognitive communication deficit, and schizophrenia, and the MDS documented severe cognitive impairment, coughing or choking during meals or when swallowing medications, and substantial/maximal assistance with meals. The resident’s care plan required extensive 1:1 meal assistance, and physician orders later specified 1:1 assistance for all meals. The resident had multiple significant weight losses over time, including a 20.4-pound loss in 3 days, a 20-pound loss over 3 months, a 12.3-pound loss in 1 month, a 49.4-pound loss over 6 months, and an 8.8-pound loss in 22 days, with several missing or undocumented reweights. Surveyors observed the resident eating without staff assistance on several occasions, and staff interviews confirmed that the resident required meal assistance but did not receive it as planned.
