Failure to Prevent Elopement for Two Residents

Summary

The facility failed to ensure adequate supervision and/or assistive devices were in place to prevent elopement for two residents identified as elopement risks. One resident had diagnoses including dementia, aphasia, and depression. After being observed unescorted and smoking in the patio area, the resident was educated about the smoking policy, refused a room search, and refused to wear a wander guard. The resident was then placed on hourly head counts and had a care plan documenting elopement risk, refusal of the wander guard, and the need for monitoring. Despite being identified as an elopement risk, the resident later left the facility without being detected. The record showed the last documented hourly head count/visual check was at 10:00 AM, and there was no documented head count or visual check after that time on the day of the elopement. Staff later discovered the resident was missing when a CNA reported the resident was not in the room during dinner. The facility initiated a search and contacted police, and a detective later confirmed the resident had gone to the representative’s residence in Manhattan and stayed there overnight. Interviews with the representative, DON, SW, RN, and Medical Director described that the resident was ambulatory, often moved around the building, and had expressed wanting to leave the facility. A second resident had diagnoses including unspecified dementia, psychotic disturbance, mood disturbance, anxiety, and bipolar disorder, and was assessed as an elopement risk with a physician order for a right wrist wander guard. The care plan directed that the wander guard be worn and checked every shift. However, during observation the resident was sitting in the dayroom without the wander guard in place. Staff stated the resident sometimes removed the device, and the wander guard was later tested and did not activate the elevator alarm when carried over to the elevator. The resident’s record also showed severe cognitive impairment and no documented behaviors on the quarterly MDS.
