Leaking Dining Room Ceiling in Maple Unit

Summary

The facility failed to ensure a safe, clean, comfortable, and homelike environment in the Maple unit dining room when a ceiling tile above a resident dining table was stained brown and later observed dripping during rain. On 06/22/2026, the tile was stained about 75% brown, and when it was raining at 4:18 PM, it was actively dripping with no receptacle to collect the water, no wet floor sign, and an approximately 2 feet by 2 feet puddle on the floor. One resident was watching television in the dining room at that time. The same ceiling tile remained bowed out approximately 3 inches on 06/23/2026 while residents were eating breakfast at the table beneath it, and again on 06/25/2026 while residents were playing bingo at the same table. A CNA stated the dining room ceiling leaked sometimes when it rained, that the table had to be moved and maintenance would remove the tile and place a garbage can under it, and that residents complained when leaks occurred during meals or activities. The Director of Plant Operations stated there had been a leak in the Maple unit dining room after the spring thaw, the roof was old, and the stained tile was the same area that had previously leaked.
