Failure to Complete Orthopedic Referral After Fall-Related Fracture

Casa Maria HealthcareRoswell, New Mexico Survey Completed on 06-25-2026

Summary

The facility failed to complete a physician-ordered referral to an orthopedic specialist for a resident after a fall-related shoulder injury. The resident had diagnoses including a history of falling, osteoporosis, and difficulty walking, and her MDS showed she was cognitively intact with a BIMS score of 14 and required supervision or touching assistance for activities. After she slipped in the shower and fell back onto a toilet seat, she later reported left shoulder pain, was assessed with pain rated 2 out of 10, and was given acetaminophen. The provider ordered monitoring and safety measures, and the resident later received a mobile x-ray for shoulder pain. The x-ray showed a left closed nondisplaced fracture of the surgical neck of the left humerus, and the provider ordered her to wear a sling, remain non-weight bearing, and be referred to an orthopedic specialist. The facility’s referral log had no documentation that the orthopedic referral was scheduled or completed. The resident stated she had not seen an orthopedic specialist and wanted to speak with one about whether surgery was an option, but she had not heard anything from the facility about an appointment. Medical Records stated nursing staff did not bring the referral to her, so she did not schedule the appointment, and the DON stated there was no evidence the referral was completed.
