For a resident with dementia, a hip fracture, unsteadiness, repeated falls, gait and mobility problems, and lack of coordination, the record showed the resident fell out of bed, but staff did not document that the IDT met to discuss the fall and the care plan was not revised afterward. For a resident with systolic CHF, the physician ordered Lasix, but the care plan did not document the use of Lasix, and an LPN confirmed the care plan was not revised to include the medication or monitoring for potential side effects.
