Summary

The facility failed to follow established menus and physician-ordered nutritional supplements for Resident #34. During lunch observation in the Grenloch dining room, the resident’s meal ticket listed pureed beef stir fry, cream of rice, pureed stir fry vegetables, vanilla pudding, fortified mashed potatoes with gravy, a high calorie frozen dessert supplement, ice cream, sherbet, and whole milk, but the resident did not receive the high calorie frozen dessert supplement listed on the ticket. Resident #34 had diagnoses including dementia, palliative care, and unspecified protein-calorie malnutrition, and the quarterly MDS dated 02/11/2026 showed a BIMS score of 4 out of 15, indicating severe cognitive impairment. The ICCP identified the resident as at risk for malnutrition and included interventions to provide supplements as ordered, including a high calorie frozen dessert supplement twice a day. The ORR showed a physician order dated 04/09/2026 for a nutritional treat/snack two times a day for planned weight gain, specifically a 4 oz high calorie frozen dessert supplement by mouth. The Dietary Director stated the supplement was not on the tray because it was a new order, and the RD stated the resident should have received the supplement at lunch and dinner to support nutritional needs.
