Summary

The facility failed to ensure professional standards of nursing practice were followed when a resident's heels were not offloaded according to the physician's order. Resident #14 was admitted with diagnoses including syncope and collapse, pulmonary hypertension, and a sprain of the left ankle. The resident was cognitively intact, required extensive assistance with care, and was identified as being at risk for pressure ulcers/injuries. A physician's order dated 4/11/26 directed staff to apply heel protectors/booties when the resident was in bed and to check placement every shift, and the care plan included heel protectors as an intervention for skin breakdown risk. During observation, the resident was seen in bed, and the assigned RN later reviewed the eTAR and stated the heel booties were supposed to be in place because her initials were documented there. However, when the RN went to the room with the surveyor, the heel booties were not observed and could not be located in the room, bedside stand, or closet. The RN acknowledged the mistake and stated the resident did not have them. The Unit Manager/RN also reviewed the record and stated the initials on the eTAR indicated the heel booties were on the resident and that it was the nurse's responsibility to ensure they were on. The facility's wound and skin care protocol stated that heels must be elevated completely off the bed surface.
