Infection Control Failures With EBP and Oxygen Equipment

Summary

The facility failed to maintain an infection prevention and control program designed to provide a safe, sanitary, and comfortable environment and to help prevent the development and transmission of communicable diseases and infections. One deficiency involved Resident #89, who had a physician order for Enhanced Barrier Precautions related to a surgical incision and a care plan noting increased infection risk and a surgical wound requiring enhanced barrier precautions. While a CNA was observed in the resident’s room providing care, the CNA wore clear disposable gloves while manipulating bed linens and removing a trash bag, but was not wearing a protective gown even though the posted Enhanced Barrier Precautions sign required gloves and a gown for high-contact resident care activities, including changing linens and providing hygiene care. During the observation, the CNA told the surveyor she had to wear a gown in the room but said she was working on the door side of the room. However, the surveyor observed the side of the room closest to the door, and no resident was present there; the bed was made and appeared undisturbed. The CNA was not observed working on that side during the surveyor’s observation. The CNA then left the surveyor’s presence holding a clear plastic bag of trash. A second deficiency involved Resident #8, who had diagnoses including acute respiratory failure with hypoxia, bronchiectasis, and adult failure to thrive, and had an order for oxygen 4 liters via nasal cannula continuously. The resident was transported into the room without the nasal cannula or oxygen in use, and the CNA stated she planned to retrieve the oxygen concentrator later because no one was available to help her transport it. The resident’s tubing and nasal cannula were observed lying across a dining room tablecloth, and the LPN then handled the tubing, placed it in a plastic bag, donned surgical gloves without performing hand hygiene, and attempted to place the same tubing back on the resident before the surveyor stopped her. The CNA stated there was usually a plastic bag attached to the concentrator for the tubing, but there was no bag that day, and acknowledged the tubing should have been discarded and replaced.
