The facility failed to follow physician orders and failed to notify the provider of required weight monitoring for two residents reviewed for nutrition. For one resident with heart failure, cirrhosis, anasarca, and acute kidney injury, the provider had ordered daily weights and notification for weight gain greater than 2 pounds in 24 hours or 5 pounds in 7 days. The record showed a 12.2-pound weight loss on one day with no documentation that the provider was notified, several days with no documented weight, a 7.1-pound weight gain with no documentation of provider notification, and two documented refusals to be weighed without documentation that the provider was notified or that a reweigh was attempted. Staff confirmed that daily weights should have been completed and that the provider should have been notified of the weight changes. For another resident, the physician ordered weekly shower/skin checks and weekly weights after admission.
