Summary

The facility failed to ensure kitchen staff performed handwashing before donning gloves and after removing gloves during food preparation and plating. During observation, Cook-A repeatedly left the prep area, returned, and put on gloves without washing hands before touching food and food-contact surfaces, including steam pans, liners, croissants, seafood salad, cottage cheese, chicken breasts, sauce, and cheese. The Director of Culinary Services confirmed staff should have handwashed prior to applying gloves and between glove changes. The facility also failed to keep kitchen equipment and surrounding surfaces clean and sanitary. Observations showed the air conditioning filters above the 3-compartment sink had a brown fuzzy substance, the top and side of the Vulcan oven had food debris and splatters, there was a brown gritty substance under the gas pipe below the oven, the shelf above the Vulcan range had scattered food debris, and the vents, coils, and wall behind the ice maker had a gray fuzzy substance. The Maintenance Director and the Director of Culinary Services both confirmed these areas were not clean and should have been. The facility failed to sanitize the food temperature probe between uses and before use. Cook-A used the probe to check multiple foods, including chicken parmesan, green beans, and buttered rotini pasta, without cleaning or sanitizing it between items, and only wiped it with a dry paper towel. The facility also failed to ensure food in storage was not expired, as the freezer contained vanilla flavored Magic Cups with an expiration date of 02/19/2026 and the bread storage area contained loaves of whole wheat bread with an expiration date of 06/14/2026. In addition, Cook-A did not follow standardized recipes for chicken parmesan and buttered rotini pasta, using unmeasured ingredients and preparing the chicken parmesan in layers rather than a single layer as directed. Finally, the facility did not ensure cold foods were temperature-checked before service; the chocolate pudding parfait was later found at 49.4 degrees F, and Cook-A confirmed the cottage cheese had not been checked before serving.
