Food Storage, Labeling, and Hand Hygiene Failures
Summary
The facility failed to ensure proper food storage and kitchen sanitation. During observations in the dry storage area, surveyors found 4 boxes of baking soda with expiration dates of July 2024, a large open and unsealed bag of macaroni noodles with no open date, and multiple frozen items in three freezers that were either unsealed, undated, or missing expiration or best-by dates. These included bags of hashbrowns, frozen peas, cinnamon rolls, chopped carrots in a blue bag, fish fillets in a Ziploc bag, ground sausage dated 5/21 and not sealed, frozen waffles, frozen bread, and frozen corn tortillas without required dates. The kitchen also had a grey fuzzy substance on the chains holding the hood vent above the stove/oven, and the ice machine lid was covered in a dry white substance with a slow water drip running down into the ice holding area. During meal preparation, the Dietary Manager performed hand hygiene with soap and water for 10 seconds before beginning fruit cups and later for 7 seconds after removing gloves. The Dietary Manager touched a paper on the prep counter with gloved hands, handled chicken from the oven, used a thermometer to check the chicken, set the thermometer down, and then reused it without recleaning it before checking the chicken again. The Dietary Manager also handled salad dressing cups, cottage cheese cups, broccoli, and sauce while moving between tasks, including leaving and returning to the kitchen and donning new gloves without hand hygiene after returning. The Cook-D also had multiple breaks in hand hygiene and glove use, including returning to the kitchen from an unknown location and donning gloves without hand hygiene, leaving the kitchen and returning with jelly packets without hand hygiene, and touching a paper on the prep counter with gloved hands while preparing sandwiches. Additional observations showed food handling and storage issues during preparation. Cook-D attempted to seal a bag of salad and found a hole in the bag; clear tape was placed over the hole and the bag was returned to the refrigerator. The Dietary Manager stated there were no pureed diets for residents, only mechanical soft. The facility policy titled Food Storage stated opened and partially used foods must be dated, labeled, and sealed before being returned to storage, and frozen foods should be covered, wrapped, labeled if not identifiable, and dated when placed in the freezer.
