Failure to Prevent and Treat a Heel Pressure Ulcer

Summary

The facility failed to implement preventative measures to prevent a pressure ulcer, failed to provide treatment as ordered to promote healing of a pressure ulcer, and failed to ensure weekly skin evaluations were completed for one resident. The resident was readmitted after a right hip fracture and had diagnoses including diabetes mellitus, liver transplant status, CHF, peripheral vascular disease, and COPD. The resident required substantial to maximal assistance with transfers and was dependent for toileting hygiene, lower body dressing, and footwear. A Braden Scale completed on 3/18/26 showed a score of 15, indicating mild risk, and the form stated that scores less than 16 required skin integrity interventions and documentation on the baseline care plan. Record review showed no interventions were implemented to prevent pressure injury to the resident’s heels, and the comprehensive care plan did not include a focus area identifying the resident as at risk for pressure ulcers or injuries. The resident’s record also showed no weekly skin/wound observation completed between 3/18/26 and 4/3/26. On 4/7/26, staff observed a 4 x 3 cm open blister on the right heel. A Tissue Analytics APRN note dated 4/10/26 documented a right heel stage III pressure ulcer measuring 4.05 cm by 3.06 cm by 0.10 cm, with slough present, and ordered wound care including cleansing, skin prep to the periwound, alginate Ag, Medihoney, dressing coverage, and gauze wrap with daily and PRN changes. On 6/1/26, observation of wound care showed an LPN removed a saturated dressing dated 5/30/26, indicating the treatment had not been completed as ordered on 5/31/26. The LPN did not apply skin prep to the periwound and did not wrap the heel with gauze as ordered. The wound was described as having a gray/yellow base with macerated edges. During interview, the LPN confirmed the dressing was not changed as ordered and acknowledged the missing steps. An LPN, the DON, and the Regional Nurse Consultant also confirmed the resident’s Braden score indicated risk and that there was no evidence interventions were put in place, and the consultant stated the facility policy required weekly skin assessments and Braden risk assessments on admission, weekly x 4 weeks, and then quarterly.
