Summary

The facility failed to ensure the care plan was complete and accurately reflected current care needs for two residents. For the second resident, the MDS showed frequent urinary incontinence and always incontinent of bowel. The DON stated the resident did not want to be in a wheelchair outside the room and did not go to the dining room or activities, but the care plan did not include the preference to eat in the room for every meal. The DON also confirmed the resident was no longer ambulatory, was no longer weight bearing, and required a Hoyer mechanical lift for all transfers, yet the care plan did not reflect these current mobility and ADL needs.
