Summary

The facility failed to administer enteral nutrition and PEG tube care according to provider orders and manufacturer instructions for multiple residents. For one resident, staff did not give the ordered amount of Glucerna 1.5 because an LPN did not measure the formula with a graduate. For another resident with a PEG tube, an LPN cleaned and applied a split sponge to the site but did not flush the tube, and the tube contained a dark brown, crusty substance inside. The DON confirmed the PEG tube should have been flushed but was not completed. The facility also left opened enteral nutrition containers in resident rooms between feedings instead of refrigerating them after opening. Observations showed opened, partially used 1000 mL bottles of Glucerna 1.5 sitting on a dresser and bedside table, including one that was warm to the touch. The DON confirmed staff were leaving opened bottles in resident rooms and were not refrigerating them after opening, and the NP confirmed the enteral feeding nutrients should have been refrigerated after opening. In addition, the facility failed to follow pressure ulcer prevention and treatment orders and did not accurately complete wound risk assessments. One resident with a right heel deep tissue injury had a green heel protector in use, but staff and the RNC confirmed the care plan had not been revised and the intervention did not match the ordered pressure-relieving device. Another resident with pressure ulcers to both feet did not receive the ordered wound dressing; staff used Opticell AG instead of Aquacel, and the NP confirmed the products were not interchangeable. The facility also failed to accurately score Braden scales for residents with impaired sensation and quadriplegia, and staff were uncertain what interventions had been put in place when one foot ulcer was first identified before the other developed.
