Summary

The facility failed to identify, assess, measure, document, monitor, and obtain physician orders promptly for a pressure ulcer that developed in a resident shortly after admission, and it also failed to develop and implement a baseline care plan for pressure ulcer prevention. The resident had been admitted after a fall at home with a hip fracture requiring surgical repair, and the hospital discharge record did not identify a sacral pressure injury. Facility admission documentation likewise stated the resident’s skin was within normal limits and did not identify a sacral wound on admission. The resident’s baseline care plan and early nursing documentation did not identify a sacral pressure injury, did not include wound characteristics or treatment orders, and did not show a comprehensive skin assessment or timely wound measurements. Although the resident was documented as being at risk for pressure injury on Braden assessments, the record showed no timely wound assessment, no physician orders for the sacral wound, and no documented wound care plan until later in the stay. Staff interviews confirmed that admission nurses were expected to complete head-to-toe skin assessments, document wounds, obtain measurements and images, notify the provider and family, and generate a baseline care plan, but staff stated these steps were not completed for this resident when the wound was first discovered. When the sacral wound was eventually identified, nursing notes described buttock pain, dressing changes, and the need for wound care consultation and a specialty mattress. The wound was later documented as unstageable and then evaluated at the wound clinic as a stage IV sacral pressure ulcer after debridement, with necrotic tissue and significant pain. The resident became increasingly uncomfortable, refused repositioning at times, voiced that she wanted to die, and was later discharged to inpatient hospice. The provider discharge summary stated that she developed a stage IV sacral wound with associated pain requiring escalation of pain management and wound care interventions.
