Summary

The facility failed to ensure that all infection control policies were updated or reviewed annually. During interviews, staff stated they believed the infection control policies had been updated in 2024 and reviewed the prior year, while another staff member stated the infection control program and policies had been reviewed two years earlier and had not been reviewed since. Record review showed several policies with outdated or missing revision dates, including the influenza immunization policy revised in 06/2017, the pneumococcal immunization policy revised in 06/2017, and the Antibiotic Stewardship Program policy with an effective date of July 21, 2017 and no revision date documented. The infection control prevention policy was last updated on 4/20/24, two years before the survey start date. The facility also failed to keep its list of reportable communicable diseases current. Staff stated they would notify designated facility staff if a communicable disease needed to be reported, and identified a document titled Reporting of Communicable Diseases in Montana dated March 2017 as the list used by the facility. Staff acknowledged the date had not been noticed and stated the list needed to be updated. Review of the Lake County health department website showed the current list of reportable communicable diseases had been revised in June 2024, while the facility was still using the older list provided years earlier.
