Dignity and Respect Not Maintained During Resident Interactions
Summary
The facility failed to ensure that staff treated four cognitively intact residents with dignity and respect when a CMT spoke to them in a harsh, demeaning, and aggressive manner and, in several instances, handled their personal belongings or provided care in a rough or disrespectful way. The cited deficiency involved Residents #4, #2, #6, and #3, out of 10 sampled residents in a facility with a census of 61. The facility policies on Dignity and Resident Rights stated that residents are to be treated with kindness, respect, and dignity, and that staff should not handle or move residents’ personal belongings without permission. Resident #4, who had cerebral palsy and was cognitively intact, reported that the CMT pulled up his/her pants too quickly and too roughly during toileting and peri-care. The Administrator documented that the resident complained the CMT handled him/her roughly, and the CMT was brought in to hear the allegation. During later interviews, the resident and a CNA described the CMT taking a fork from the resident’s hand at lunch without asking while the resident was trying to cut food, speaking in a loud and scolding tone, and leaving after saying the resident would be reported as refusing help. The resident stated the CMT often worked too fast, argued when told to slow down, and made him/her angry. Resident #2, who was cognitively intact, said the CMT entered the room without knocking, grabbed a blanket from a chair beside an empty bed, and tossed it onto the resident’s bed while telling the resident to keep belongings on his/her own bed because the other side of the room needed to stay clean and ready for a possible new admission. The resident described the tone as harsh and aggressive and said it felt degrading and like being treated like a child. The resident’s family member and an LPN also described the CMT’s tone as abrupt, short, and scolding. Resident #6 reported that the CMT was rude and hateful when the resident asked for assistance at the nurses’ station, speaking in a harsh tone and saying there was no time to help. Resident #3 reported multiple incidents in which the CMT did not knock before entering, pulled up a shirt to place a pain patch without speaking, responded harshly when the resident asked about medication timing, and took items from the resident’s hands while telling the resident to go back to his/her room, which made the resident feel like a child being scolded.
