Summary

The facility failed to provide adequate treatment and services for two residents with pressure ulcers and failed to carry out pressure-ulcer prevention interventions for one resident. One resident had severe cognitive impairment, was fully dependent for ADLs and mobility, was frequently incontinent, and had multiple pressure injuries including a stage 4 sacral ulcer, stage 3 ulcers, and an unstageable ulcer. The care plan called for a low air loss mattress, pressure-reducing devices, daily skin inspections, and wound treatments, but surveyors observed the resident lying on a folded blanket, linen pad, and brief, with the LAL mattress set at varying weights that did not match the resident’s current weight. Protective boots were not consistently on the resident’s feet, and the resident’s wound dressings were observed soiled, dated illegibly, or not changed when drainage had bled through. The resident’s wound record showed worsening pressure injuries over time, including enlargement of the sacral wound and development of additional wounds on the right knee, right buttock, right upper back, right thumb, and foot. The wound physician documented repeated deterioration and noted that the resident had several new wounds and a continued decline in overall status. After hospital discharge, wound orders required BID dressing changes for the sacral, hip, buttock, and other wounds, but the MAR/TAR showed missed treatments for the sacral wound and left ischial wound. Surveyors also observed the resident positioned on pressure-contributing items such as a folded blanket, linen pad, and brief, while staff interviews showed inconsistent understanding of offloading, LAL mattress settings, and the need for bilateral boots. A second resident had dementia, diabetes, dysphagia, and pressure wounds to the sacrum and left hip. The wound physician’s notes showed the sacral wound worsening and then fluctuating with debridement and multiple dressing changes ordered, including daily and PRN changes when saturated, soiled, or dislodged. On observation, the resident’s left hip dressing was undated, and the sacral wound was found uncovered with yellow and light green drainage, necrotic tissue in most of the wound bed, and surrounding redness and inflammation. A CNA stated the resident had been changed once during the night shift and the sacral wound was uncovered. The report also noted that the resident was very thin and had wounds to the left hip and sacrum that should have been covered.
